1/22/2023 EnchantaVaughn Approved 1. Behavior: Upon visiting the client to complete his home therapy, it was reported by mom he was not in a good mood. Client appeared to be tired and underwhelmed with other obligations. Client reported he does not sleep well at night because his siblings make so much noise and don't give him enough space in the room. Client looked exhausted and his movement was slow in range. On top of that, client's depression seems to make him more tired and lazy at home. Even though, he is not sleeping during the correct times of the day. 2. Behavior: Client experiences depression daily, occurring 3-4x a day lasting 30-45 minutes. Most depressive symptoms are related to mental and emotional difficluties and issues with home life. He was not able to maintain control of conversations or maintain authority prompts and requests. Client experienced moments of frustration. These levels of frustration increased when it came to client's interaction with his family. Client's frustration level is reported to be moderate in level and occurs daily, lasting 20-30 minutes at a time until he goes to sleep. 1. Intervention: QMHP facilitated one on one session with client to review being kind to others and not letting his emotions get the best of him. QMHP administered the session to enhance clientâs ability to find compassion for his loved ones or learn a new skill that will allow him to be a more stable and understanding person. QMHP explained to client that we all have moments we want to be alone but sometimes itâs good to be around people who care about us when we arenât in the best state. 2. Intervention: QMHP encouraged client to be kind to himself, his peers, and his elders. QMHP explained to client that people always remember how you made them feel. QMHP informed client that we need friendships and positive relationships so that we learn how to give, accept, and receive constructive criticism and support from others. QMHP educated client that having a strong support system can uplift our mood, reduce stress and anxiety, improve our self-esteem, and hold us accountable for our actions. 1. Response: Clientâs mother sat and listened as the treatment plan goals and the plan of actions were read off to her. Clientâs mom made no objections to the treatment plan as she signed it and agreed to the terms and conditions. The interventions were viewed as successful, as seen by clientâs open and honest affirmations when expressing his point of view. Client was compliant with following along with the QMHP directions. Client was able to demonstrate the correct emotions while presenting appropriate manners towards the QMHP. 2. Response: Client was receptive to the interventions, as he was able to provide positive responses during the session. Client was able to identify examples of how to be kind and was very cheerful, at this time. Client was observed completing the therapeutic activity in a timely manner and displaying a positive attitude. Client shared that he tries not to blame others for his negative actions. Client appreciates his family for supporting him with all of his needs and is thankful for the people that genuinely love and care about him.
